420 fps semi auto only 📍 Martinez Memorial Hospital - 4th floor PARTICIPANT ACCIDENT WAIVER, ASSUMPTION OF RISK, AND RELEASE OF LIABILITY For participation in an airsoft event conducted at the designated Martinez Colleges, Inc. (MCI) venue. Complete and sign BEFORE entering the playing area. 1. Acknowledgment of risks I understand that airsoft involves simulated combat, airsoft replicas, BB projectiles, physical exertion, running, obstacles, uneven surfaces, slips, falls, collisions, and other inherent and foreseeable hazards. Potential injuries include eye or skin injuries, bruising, sprains, fractures, and serious injury. I voluntarily choose to participate with knowledge of these risks. 2. Safety rules and fitness to participate I agree to attend the safety briefing; wear approved eye protection and all required protective equipment at all times in the game area; comply with chronograph/FPS limits, safe zones, marshals’ instructions, and venue rules; and refrain from participation when impaired or medically unfit. I will promptly report unsafe conditions, injuries, and incidents. I understand I may be removed from play for violations. 3. Limited release and assumption of risk To the extent permitted by Philippine law, I assume the ordinary risks inherent in voluntary airsoft participation and release the ORGANIZER and Martinez Colleges, Inc. (MCI), together with their respective directors, officers, employees, and authorized representatives, from claims arising solely from those inherent risks where no actionable fault is attributable to the released party. This release does NOT waive claims for a party’s own negligence, gross negligence, willful misconduct, or other liability that cannot lawfully be excluded. 4. Medical assistance and emergency response In an emergency, I authorize the ORGANIZER to contact emergency responders and my emergency contact and to arrange reasonably necessary medical assistance when I cannot provide consent. I understand that medical costs and responsibility for injuries will be determined according to applicable law and the facts of the incident; this form is not an agreement to excuse any party’s legal liability. 5. Personal property and incident reporting I will secure my personal belongings and immediately report any accident, injury, or property damage to the event marshal. Responsibility for any loss or damage remains subject to applicable law. 6. Personal information and waiver records I acknowledge that the ORGANIZER will process my identifying, emergency-contact, attendance, and signed-waiver information to administer the event, manage safety and incidents, and establish or defend legal claims, in accordance with its privacy notice and applicable Philippine data privacy law. Relevant records may be shared with MCI, emergency responders, insurers, or authorities only where lawful and necessary. I may request the ORGANIZER’s privacy notice and contact details. 7. Participant confirmation I have read and understood this document, had an opportunity to ask questions, and sign voluntarily. I understand that signing is a condition of participating but does not eliminate any non-waivable legal rights.
